The IHSAA Pre-participation Physical Evaluation (PPE) is the first and most important step in providing for the well-being of Indiana's high school athletes. The form is designed to identify risk factors prior to athletic participation by way of a thorough medical history and physical examination. The IHSAA, under the guidance of the Indiana State Medical Association's Commitee on Sports Medicine, requires that the PPE form be signed by a physician (MD or DO) holding an unlimited license to practice in the State of Indiana.
We offer IHSAA Pre-Participation Physical Evaluations (PPE) for your student athlete. Has your student althete lost his/her form, forgotten to pick one up, or left it in his/her locker? No need to worry, we have copies of the PPE form in our office just in case.
The cost of the IHSAA PPE sports physical is $50. This service is "self-pay" only; we do not accept any form of insurance for Pre-Participation Physical Evaluations. Check with your student's school to avoid missing the deadline for turning in your physical for sports participation.
